Clients are able to book online with any of our therapy team members using our online appointment booking portal. However, due to the fact that Sport and Exercise Medicine Physicians are covered by OHIP, a referral from a physician or primary care provider with an active OHIP billing number is required in order to be seen. Unlike our rehabilitation team members, who operate on a fee for service basis, Sport and Exercise Medicine Consultations are are covered by OHIP.
getting referred by a physician
If you would like to see one of our Sport and Exercise Medicine Physicians, you will require a referral letter from another physician as this is a consultation service rather than primary care or emergency medical care. There are several people who can do this for you:
A family physician
A nurse practitioner
An emergency room physician
A walk in clinic physician or nurse practitioner
Any other physician with an active Ministry of Health OHIP billing number
If they do not have our referral form, a form can be printed off, filled out, and faxed back to 705-789-1509. Our Sport and Exercise Medicine Physicians focus on residents (permanent, visiting, cottagers) of Muskoka, however, they will accept referrals from anywhere in Ontario. Referrals will not be accepted for cases associated with motor vehicle accidents, insurance claims, or WSIB.
getting referred by a therapist or an allied health professional
Sometimes, your therapist wants clarification on a diagnosis so that they can better target their treatment plan, and would like to have one of our Sport and Exercise Medicine Physicians clarify the diagnosis with a thorough history, physical, and possibly tests that could include blood work or imaging. Additionally, an injection (steroid, prolotherapy, or other) may assist with pain and mobility during your rehabilitation treatments. In order to have OHIP pay for the consultation, the therapist or allied health professional needs to contact a physician to initiate a referral.
In order to simplify this, we ask that you print off this form that can be printed out and sent to a physician who can either:
approve the referral request, after which they sign the letter that was sent to them, and then they fax it to us at 705-789-1509
ask for the patient to come and see them first in their office so that they can do a preliminary screen or order tests before making the decision to refer.
